Product Description

The Fisher Paykel Forma full face CPAP mask is made for the consumer that breathes in and out through their nose and/or mouth during the use of a CPAP or BiPAP machine. Obstructive sleep apnea (OSA) occurs at the beginning of the inhalation and some customers find their pressures to be at a point where they need to use either their nose or mouth or both to handle the pressures being provided by the CPAP or BiPAP machine.

The Forma is designed to fit the user from the bridge of the nose to under the chin. The lower part of the mask will actually rest under the user's chin so that if you experience your chin dropping during the night due to it relaxing, the mask will still hold its seal. This feature eliminates the need for a chinstrap as some people will still need a chinstrap added with some of the other manufacturer's masks.

The Forma full face also has an excellent feature known as glider straps that will allow the person wearing the CPAP mask to move freely during their sleep. This reason for the freedom of movement is that the glider straps slide along grooves at the front of the mask so that when your headgear is caught between your head and pillow and you move your face, the mask still moves with you.

What Style of CPAP Mask is This?

This is a full face style CPAP mask. These masks use a silicone cushion that provides a seal around the
nose and mouth to allow the user to breathe through both.

Will This Brand
of CPAP Mask Work with My CPAP or Bi-Level Machine?

Most CPAP masks are designed
to be universal and will work with any manufacturer. Example: Your Philips
Respironics CPAP mask will work with your ResMed CPAP machine. The masks are
also designed to fit any brand of standard 22mm CPAP tubing. The exception to
the rule will be for certain travel CPAP machines like the ResMed AirMini
whereas special masks and connectors are needed.

I Want to Submit a Self-Claim to My Insurance. Can I Do
This?

In most cases you can submit a claim form for an out-of-net
network provider. You may receive reimbursement back from your provider
depending on your coverage and co-pay. The HCPC code for a full face CPAP mask
is A7030 and A7035

How Often Should I Replace This Mask?

This will depend on how well you clean and maintain your
CPAP mask. The typical lifespan for the complete mask is 3-12 months based on
care. The cushion seal and headgear should be replaced every 1-3 months based
on wear and tear. Signs to replace will include noticeable mask leakage and/or
discoloration.

How Do I Clean This Mask?

You should consult the owner’s manual that is included with
your mask system. The rule of thumb for most CPAP masks is to clean with mild
soap by hand and let air dry. It is important to clean your cushion seal daily
to remove facial oils, makeup or dirt that can cause mask leaks. Your headgear
strap should be cleaned by hand weekly and left to dry by room air.

This mask has been the best for me. I first had the FisherPaykel CPAP and when that went out my roommate had the Flexfit432 and I used that then went to the Forma and it fits like a glove. I'm a tummy sleeper so it doesn't leak when I'm sleeping
