Do I Need a Prescription for This Product?

No, mask parts do not require a prescription for purchase. Often
you can prolong the life of your CPAP mask by replacing the headgear, a great
cost savings!

How Do I Know Which Headgear Size to Buy?

It is very important to confirm the correct model and size
of replacement mask headgear. 1.) While most CPAP masks use a standard headgear
size, some offer different sizes. 2.) Once a headgear has been opened from the original
packaging it cannot be returned. You can usually find the size of the headgear labeled

How Often Should I Replace My Headgear?

Your mask cushion seal typically has a life span of 1-3 months,
however with proper care they can last up to 6 months. Signs of replacement will
be excessive mask leak and/or discoloration.

How Do I Clean My Mask Headgear?

You should consult the owner’s manual that is included with
your mask system. The rule of thumb for most headgear straps is to handwash and
let air dry. DO NOT PUT HEADGEAR IN DRYER. It is important to clean your headgear
weekly to remove oils, makeup or dirt that can lead to mask leaks.

I Want to Submit a Self-Claim to My Insurance. Can I Do
This?

In most cases you can submit a claim form for an out-of-net
network provider. You may receive reimbursement back from your provider
depending on your coverage and co-pay. The HCPC code for a CPAP mask headgear is
A7035.

This mask has been the best for me. I first had the FisherPaykel CPAP and when that went out my roommate had the Flexfit432 and I used that then went to the Forma and it fits like a glove. I'm a tummy sleeper so it doesn't leak when I'm sleeping

I have tried multiple cpap masks over the years but this one really worked for me. it sets below my chin which helped me from my mask leaking by keeping my chin up. I have a beard so if you have a beard or your mouth is dropping open, you should try this
